The Canadian Forces National Investigation Service Western Region (CFNIS WR) investigated two sexual assault allegations made in November 2021 against a Canadian Forces officer posted to 15 Wing CFB Moose Jaw, Saskatchewan. Following a two-day investigation, the officer was arrested and charged with two counts of sexual assault; he died by suicide on January 17, 2022. The Military Police Complaints Commission conducted a Public Interest Investigation and found the CFNIS WR investigation was inadequate, marked by undue haste, confirmation bias, and tunnel vision. The CFPM declined to implement any of the 13 recommendations issued by the MPCC Chairperson in the final report dated February 4, 2026.
